IN PERSON

You may download and complete the registration form in advance by clicking: Printable Registration Form, or simply stop into the recreation center.

• If you are registering for an ice rink related program, go to the Ice Center located on the first floor of the Mt. Lebanon Recreation Center.
• All other programs proceed to the second floor Recreation Office.
• Put ice rink programs on a form separate from other recreation programs.
• Some programs will have a special registration form which will be included with the flyer.
• Methods of Payment – Checks (made payable to “Mt. Lebanon, PA”), Visa, MasterCard or Cash
